Disease state management protocols and programs are designed to effectively manage patients with chronic conditions. The Joint Commission on Accreditation of Healthcare Organizations has established a disease certification program as part of its offerings, but with the recent announcement regarding the voluntary collaborative effort between the American Hospital Association, the American Association of Medical Colleges, and the Federation of Hospitals, new emphasis is bound to be forthcoming.
