Case management as a day-to-day activity is limited to very few types of nurses, although it is a critical component to home health, hospice care, and other community health positions. It is one of the more difficult tasks to achieve when orienting to the community health environment (probably second to the infamous paperwork). The concept of being responsible for the patient 24 hours per day, 7 days per week even if you are off or not present in the home can be daunting. The measure of satisfaction with an agency is often dependent on the patient’s and family’s perceptions of how well “Susie the registered nurse” is taking care of all of their needs. Some commonly recognized attributes for Susie are as follows.
