Cardiovascular disease (CVD) rank as the first cause death in the world, especially in industrialized countries. In Indonesia, CVD become increasingly prevalent from year to year. The cause of CVD is multifactors, o.a. hyperlipidemia characterized by high level of Low Density Lipoprotein (LDL) and triglyceride in the blood, also accompanied by low level of High Density Lipoprotein (HDL). LDL is atherogenic after oxidation process by free radicals. Oxidized LDL products are cytotoxic to endhotelium, .fibroblasts and smooth muscle and cause the dysfunction of endhotelial system leading to atherogenesis. The increasing oxidized LDL level in the blood increase atherosclerosis risk. CVD can be prevented by reducing blood LDL and triglyceride level, and increasing blood HDL level and preventing LDL from being oxidized. Antioxidant can prevent and inhibit atherosclerosis by: 1). Inhibiting atherogenesis by reducing Vascular Cellular Adhesion Molecule-l (VCAM-l) expression; 2). Lowering serum cholesterol level by inhibiting HMG-CoA reductase enzyme activity, reducing glucose-6-fosfatase dehydrogenase (G6PD) level and isositrate dehydrogenase, increasing lipoprotein and LCAT; 3). Inhibiting oxidation of LDL; 4). Increasing blood HDL level; 5). Lowering blood triglyceride level; 6). Lowering TEARS (Thiobarbituric Reactive Substances) level.
