The clinical course of syphilis can be divided into three stages (for a detailed clinical review, refer to the 2006 Centers for Disease Control and Prevention [CDC] guidelines'). Primary syphilis is characterised by a single genital or, occasionally, extragenital indurated ulcer within 3 weeks after infection (the chancre), accompanied by regional lymph-node swelling in 70-80% of cases. The classic chancre is seen only in 60% of patients and is usually painless, but can also be painful or non-indurated. At this stage, the diagnosis is established by dark-field microscopy or direct fluorescent antibody test of lesion exudates.
