Secondary hyperparathyroidism is seen in most cases of chronic renal failure, yet rarely requires operative treatment. The pathophysiology is complex, involving the kidney, liver, intestine, and bone in addition to the parathyroid gland. Although medical management suffices for most cases, the remainder require parathyroidectomy. The indications for operation and extent of treatment are discussed.
