Isi artikelRadiation therapy for urologic, gynecologic, and colorectal cancers may cause chronic rectal and small bowel injury. The former is manifested as rectovaginal fistulas, anorectal strictures, and hemorrhagic proctitis; the latter by obstruction and fistulas to adjacent organs. Surgical correction may be considered only after recurrent cancer has been excluded and an assessment of rectal function and continence performed. New management approaches include abdominopelvic partitioning, radioprotective medications, formalin instillation, and vascularized colonic grafts. Fecal diversion, as the sole method of treatment, still has an important role in the management of selected patients unfit for resectional or reconstructive procedures.
