Extended highly selective vagotomy, at present, is considered by most authorities to be the best avenue for the treatment of peptic ulcer disease and to be more specific for the treatment of duodenal ulcer. Knowledge of the anatomy of the vagus nerve and its vagaries is essential for any kind of procedure the surgeon will perform. This article presents the vagal anatomy with its variations as well as the philosophy of the extended highly selective vagotomy.
