Orthotopic liver transplantation is the most formidable procedure performed in general and transplantation surgery today. No operation requires a greater knowledge of anatomy, and no operation is so unforgiving. A debt is owed to the pioneers of this procedure, and “first among equals” is Thomas Starzl. This article chronicles the accomplishments of Starzl and his colleagues and details the anatomic variations of hepatic segmental, arterial, venous, and ductal anatomy, knowledge of which is the first step in the successful completion of this challenging procedure.
