With the rise of laparoscopic surgery, most cholecystectomies now are performed with the laparoscope. The technique of open cholecystectomy has fallen out of practice, and for some surgeons in training, has never been incorporated as part of their surgical armamentarium. The authors try to impart an appreciation for a safe open cholecystectomy and suggest ways of dealing with the more complicated operation.
