An increasing number of patients will be seen with abdominal aortic aneurysms, carotid artery disease, and severe peripheral ischemia in the future. Although advanced age has been considered a strong contraindication to the surgical treatment of these conditions, an objective analysis of the available evidence does not support such therapeutic conservatism. Clinical judgments should be based on the patient’s physiologic, not chronologic, age. Specifically, properly selected elderly patients with aortic aneurysms, carotid disease, or limb-threatening ischemia can undergo surgery with acceptable risk and significant long-term benefit.
