The elderly are a heterogeneous population but, in general, they have limited physiologic reserves and less effective mechanisms to restore and maintain homeostasis. Thus, the clinician must be particularly vigilant in searching for signs of early organ system dysfunction and providing appropriate support. Priorities of care and overall approaches are similar in the elderly patient. This article reviews the factors that affect the outcome of elderly patients with critical illnesses, and discusses how these factors may modify management of these patients in the intensive care unit.
