The incidence of breast cancer increases with age. In spite of competing causes of mortality, breast cancer is a significant cause of death in elderly women. Elderly women are more likely to receive inadequate local and systemic therapy than their younger counterparts. Mastectomy and breast-sparing surgery with radiotherapy are well tolerated by the majority of elderly women. Tamoxifen is an acceptable alternative primary treatment for those with a limited life span. Treatment decisions for older women with breast cancer should be made based on biologic, not chronologic, age.
