The evaluation and surgical treatment of primary hyperparathyroidism and thyroid nodules are discussed, with particular emphasis placed on the presentation and treatment of these disoiders in elderly patients. Primary hyperparathyroidism should be treated surgically, except in patients who are absolutely asymptomatic. In addition, the postoperative regulation of calcium homeostasis is essential. Thyroid nodules should be evaluated preoperatively, with the goal of obtaining a histologic diagnosis. Based on this diagnosis, elderly patients then are candidates for either surgical or medical treatment or a combination of both.
