Surgery in the elderly should be directed at providing the best possible care to the patient at the lowest physiologic cost. Minimally invasive surgical techniques are attractive in that they accomplish these goals and return the patient to his or her familiar environment rapidly. Laparoscopic cholecystectomy, in particular, has been shown to be associated with substantially lower morbidity and mortality than open cholecystectomy in a comparable age group. Patients older than 65 years can be discharged at a median of 24 hours and are able to return to their preoperative level of activity at a median of 7 days after surgery. Unfortunately, in comparison with younger patients, the elderly are twice as likely to require conversion to laparotomy usually as a consequence of the chronicity of the disease and the increased likelihood of presenting with complicated biliary tract disease, especially acute cholecystitis.
