An understanding of gastroesophageal reflux disease (GERD) and associated conditions is important in the surgical treatment of GERD in infants and children. Special emphasis is concentrated on the method of esophageal pH monitoring used for the diagnosis of GERD, the role of GERD in sudden infant death syndrome, Barrett’s esophagus, and treating the neurologically impaired child. The three antireflux operations used most frequently by the author can be tailored to the child with GERD based on clinical findings as well as on objective tests for GERD and related alimentary tract motility disorders. Long- term results and complications of antireflux operations in childhood also are presented
