Abdominoperineal excision of the rectum is the only curative option for treating most patients with carcinoma involving the distal rectum. Local excision, electrocoagulation, and intracavitary radiation therapy are effective alternatives for the carefully selected patient with small early stage carcinoma of the rectum. The criteria for patient selection and the technique for local treatment of rectal carcinoma are reviewed.
