Major gastrointestinal surgery has the potential to cause significant nutritional deficiencies, emphasizing the risk of even short-term starvation. Some patients will present malnourished as a consequence of underlying disease. Others will develop malnutrition as a result of the hypercatabolism associated with operative complications and prolonged illness. The elderly and those with significant morbidity are at the highest risk. In addition, some surgical alterations of the gastrointestinal tract will result in long-standing nutritional deficiencies. Skills need to be developed to identify the high-risk patient before surgery, and practice guidelines for nutritional strategies can then be used to control morbidity, disability, and mortality.
