The authors provide an overview of the changes in amino acid metabolism that follow surgery and infection. As our knowledge of the regulation of these alterations improves, the design of newer and more efficacious feeding formulations will follow. The use of specific amino acids, which may have to be provided in pharmacologic doses, may improve cellular nutrition and function in surgical patients. Such specialized substrate delivery may become a pivotal part of nutritional care in the future.
