Fluid and electrolyte management in the perioperative and postoperative periods is still a challenge for those physicians who do not regulary care for seriously ill children. This article uses the scientific basis for fluid administration to derive a program that accounts for maintenance fluid needs, measurable external losses, and third-space shifts as well as addressing electrolyte needs in both the normal and abnormal state. Finally, a practical outline of nutritional requirements is presented.
