The management of malignant mesothelioma remains a controversial topic. Earlier diagnosis, an improved staging system, multimodal therapy, and collaborations between medical centers have advanced treatment strategies for mesothelioma. The maximal response rate seen with any single chemotherapeutic agent has been 20%. Radiotherapy is being used for adjuvant therapy and palliative management. Two surgical operations in practice for diffuse malignant mesothelioma are pleurectomy with or without decortication and extrapleural pneumonectomy. Other modalities being explored for treatment include gene therapy, immunotherapy, immunoconjugate therapy, chemohyperthermia and photo-dynamic therapy Further research to determine the optimum neoadjuvant and adjuvant regimen is ongoing.
