Human papillomavirus (HPV)-include lesions span a wide clinical spectrum from benign lesions to precancerous lesions, with only a minority of infections resulting in invasive cancers. Unfortunately, the prevalence of HPV infection is so widespread that the incidence of low- and high-grade squamous intra-epithelial lesions (HSIL) has doubled in the last decade resulting in large societal and personal costs. The recent gain in knowledge of HPVs genomic function has led to a greater understanding of the natural history of HPV infection in women starting from infection to the development of invasive cancer. It is now clear that LSIL is a benign HPV-associated lesion and could best be observed in young women. On the other hand, HPV infections during adolescence appear to be a risk for the future development of HSIL and invasive cancers supporting the need for close surveillance in women who initiated intercourse at a young age. Last, new self-applied therapies show promise in adolescents but their use in the age group requires closer evaluation.
