The detection of an adnexal mass in an adolescent raises concerns about surgical therapy and possible malignancy. Patients and their families can be reassured that the majority of these lesions are benign and that many may resolve spontaneously without surgery. Ultrasound is critical for diagnosis, management decision making, and follow-up of patients with adnexal masses. If surgery id required, conservation of reproductive and endocrine function can be achieved in most cases.
