For many congenital immunodeficiency disorders, stem-cell transplantation is curative. The decision to proceed with this potentially dangerous treatment depends on the severity of the underlying illness. This article discusses the mechanics of allogeneic stem-cell transplantation and the clinical experience to date in the treatment of immunodeficiency disorders. The recent advent of nonmyeloablative, stem-cell transplantation also is presented in the context of clinical trials examining the efficacy of this less-toxic approach to stem-cell replacement for immunodeficiency disorders.
