A child with a history of recurrent, prolonged, severe, or unusual infections presents a diagnostic challenge to the pediatrician. In this article, the authors concentrate on presenting to the practicing physician a systematic, step-by-step approach to the evaluation of a child with recurrent infections. The common causes of frequent infections during childhood, pertinent components of the history and physical examination, and an appropriate choice of screening laboratory tests are discussed. Significant advances within the past decade in our understanding of the molecular and genetic mechanism of these primary immunodeficiencies have enabled early recognition and more options for treatment.
