This article reviews the current management strategies for the treatment of acute hypoxic respiratory failure. These include inhaled nitric oxide, exogenous surfactant therapy, high-frequency ventilation, permissive hypercapnia, and prone position ventilation. The place of extracorporeal membrane oxygenation in the event of the failure of these therapies is discussed.
