This article reviews the evidence base of the pharmacologic treatment of psychosis and agitation in dementia and Parkinson's disease, focusing on the potential benefits versus risks for each major class of drug that has been studied. Dementia and its behavioral complications, are chronic conditions, and the need to guide treatment should include information that goes beyond the efficacy of individual agents in relatively short-term studies to include findings that validate algorithms for combining or sequencing treatments over time, depending on the individual patient's responses. However, until now, little research has been conducted to address questions about long-term management.
