Couples presenting for couples therapy where both partners manifest significant psychopathology, particularly personality disorders, are not well served by systemic marital therapy approaches. However, a treatment approach that focuses on the interlocking personality disorder dynamics of both partners in a systemic treatment context increases the likelihood of positive treatment outcomes. Such a therapeutic approach is described and illustrated with an extended couples therapy case.
