Health reforms in Indonesia need health services to be having more access, equity, quality, efficiency and sustainability. With accordance to that, indonesia needs new mindset, new paradigm in managing health system, especially by conducting of Law No. 22 and 25 year 1999, concerning health decentralization. The aim of this model was to arrange national and province health system that can be used as a basis for Health Municipal District Planning, Method for developing the model was based on the results of Symposium and Best Practice in Public Health I : Surabaya, 8 - 10 November 2001. The model was made upt through SWOT Analysis and Strategic issues. The results revealed that National and Province are the Supra System of Health Municipal Districts. Health Municipal District can plan medical care, health manpower and its financing. Health Municipal District System Plan Services are based on specific local condition besides national health policies...
