The revised American Joint Committee on Cancer (AJCC) staging and classification system for melanoma now cites 3 variables as having the greatest prognostic significance in thin melanomas; depth, ulceration, and the presence of mitotic figures. Melanoma pathologic staging has been revised to reflect these variables, with thin melanomas (=1 mm in depth) that lack mitoses or ulceration staged as pT1a, and thin melanomas with either ulceration and/or mitoses staged as pT1b.1 Patient treatment is affected by this stratification, as patients with pT1b melanoma often undergo sentinel lymph node biopsy.
