Isi artikelOrogastric tubes are commonly used in newborns to facilitate feeding, administration of medication and stomach decompression as some infants sucking reflexes are not well establish. Incorrect orogastric tube placement can lead to serious complications such as aspiration pneumonia, malabsorption resulting in weight loss or mortality. This has prompted us to review orogastric tube placement and improves the method of securing orogastric tube. The project aim is to reduce incidence of orogastric tubes misplacement in newborns to zero at Neonatal ward within 12 months. We conducted daily random survey on incidences of orogastric tube misplacements in 42 infants and brainstormed underlying causes that lead to incidences of orogastric tubes misplacement. A structured Plan-Do-Check- Act (PDCA) cycle was applied. A standardized technique on obtaining accurate orogastric tube internal length measurement and revised method of securing orogastric tube placement was adopted. Before implementation, the percentage of time infant’s orogastric tube misplacement was between 10 and 40%. After implementation, the incidence of orogastric tube misplacement was reduced from 4.5% to zero. The implementation of change in clinical practice was associated with a significant decrease in the incidence of orogastric tube misplacement, thus reduced the risk of complications, length of stay and hospital cost.
