Abstract
Background: Stroke is the third leading cause of death worldwide and the most frequent cause of disability in developing country. From all of the stroke cases, around 87% are ischemic and the rest (13%) are non-ischemic (hemorrhagic). Studies have shown that mechanism of stroke is often related to atherosclerosis, which can be caused by various factors, one of them is cholesterol levels.
Objective: Determine the relationship between lipid profile and the occurence of different types of stroke in stroke patients with hypertension in Atma Jaya hospital.
Method: This cross sectional study was implemented through data collection that have been conducted in stroke registry with participants included stroke patients admitted to Atma Jaya hospital from January 2014-December 2014. The records contain patient characteristics such as age and sex, blood pressure, types of stroke, total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. In this study, two case groups were undertaken: 55 subjects with cerebral infarction and 23 subjects with cerebral hemorrhage. Result: Subjects experiencing an ischemic stroke were found to have higher levels of total cholesterol and LDL-C compared to hemorrhagic subjects. Hypercholesterolemia were found higher in female subjects than male. Chi square test between total cholesterol and types of stroke were p= 0.008 (OR=3,85) and p=0,021 (OR=3,20). In grade II hypertension subjects p-value for association between total cholesterol, LDL-C, and HDL-C were p=0,030 (OR=3,60) and p= 0,018 (OR=4,20). In female patients p value between total cholesterol, LDL-C, and HDL-C were 0,001 (OR=10,50), 0,020 (OR=5,00) and 0,009 (OR=12,00)
Conclusion: Total cholesterol (p=0.008) and LDL-C (p=0.021) were significantly associated with ischemic stroke. In grade II hypertensive subjects, odds ratio between LDL-C and ischemic stroke was increased. Odds ratio between total cholesterol and LDL cholesterol in female subjects were higher than in both gender, also positive association was found between triglycerides and ischemic stroke in female subjects.
