In higher education, included medical education (basic, postgraduate, and continuing professional develop¬ment), innovations and changes should be carried out continuously to keep pace of science and technology (bimolecular technology) development. Lesson learned from the development of medical education, health care, and health financing systems in ASEAN countries, and based on the Indonesia real current condition, a National Policy in accelerating medical specialist care and education or training have to be initiated. The various needs and demands of our own (Indonesia) community, particularly the disadvantaged group, have to be the main priority target. The issues of regional and global competitions, e.g. tertiary medical care and education or training, should also be considered. The role of stakeholders, based on the principle of Good Governance, are responsible includes Regulator (Department of Education, Department of Health, and Department of Internal Affairs, Indonesian Medical Council, Professional Association/Colloquiums, and District Ministry of Health), User (Private and Public Institutions), Educational Institutions (Faculty of Medicine and Teaching Hospitals), and Funding Agencies (Central Government, Provincial and District Government, Foundations), should be clearly defined. The acceleration programs in East Nusa Tenggara (5 year evaluation) and HWS (2 year evaluation), seem not yet response to the problems, needs and demands of medical specialist services in these peripheral areas. The alternative models or grading or standards of specialist education should be developed and must be supported by all stakeholders as the Indonesian National Policy.
