Isi artikelMany clinicians are questioning the need to restore or maintain sinus rhythm with anti-arrhythmic drug or electrical cardioversion in patients with atrial fibrillation (AF), after four recent trials suggested that a strategy of merely controlling the ventricular rate produces equivalent result. The rhythm control strategy was associated with more hospitalization and higher risk of other arrhythmias. The decision to choose rhythm or rate control strategies should be individualized and depends on the expected benefit of restoring sinus rhythm, chance on failure to maintain sinus rhythm in the long-term, and the like hood of adverse drug effects. In all patients with AF, treatment should focus on underlying heart disease, anticoagulation, and control of ventricular rate during AF. Rhythm control remains first choice for patients with first episode or highly symptomatic episode of AF and for patients who have AF caused by reversible cause (e.g. hyperthyroidism, post-cardiac surgery) or who have a high chance of remaining in long term sinus rhythm (young patients, no hypertension, normal left atrium size, short preceding AF duration)
