Background: Cigar cells and cells shaped like thick cigars often term pencil cell which are found in peripheral blood film may be numerous in iron deficiency anaemia (IDA). However, some abnormal shaped erythrocytes in few numbers can be found in normal individual. Commonly, those cells are reported qualitatively because there is no quantitative limit range indicated IDA. Therefore, it is important to know to what extent an amount of cigar and pencil cells indicate IDA? In addition, there are no data about the association these cells with iron profile, erythrocytes index and classification of IDA. Objectives: 1) to know the correlation between pencil and cigar cells with iron profile, erythrocyte index and level of iron deficiency anaemia; 2) define the amount of pencil and cigar cells indicate IDA Methods: A cross sectional study was carried out in the study. Patients who met the eligible criteria i.e. adult patients with anaemia who have result of pencil and cigar cells on blood smear, complete blood count and iron profile, involved in the study. Data was taken from April until July 2004 in Haematology subunit, Clinical Pathology Laboratory, Sardjito's general hospital. The blood smear slides which are routinely saved before were collected and examined by counting the pencil and cigar cells. Pearson test was used to test the correlation between pencil and cigar cells with iron profile, erythrocyte index and level of IDA Result: We found 60 subjects; consist of 34 women and 26 men. Pencil cells have a good correlation with Total Iron Binding Capacity (TIBC) (r=0, 75; p<0,05), ROW (r=0, 77; p<0, 05) and IDA state (r=0, 83; p<0, 05). However, there is a poor correlation between cigar cells with TIBC (r=0, 30; p<0, 05) and ROW (r=0, 40; p<0, 05), respectively. But, moderate correlation is shown in the association between cigar cells and level of IDA (r=0, 50; p<0, 05). The amount of 2-3 pencil cells/immersion field and/or >4 cigar cells/immersion field in blood smear are found in patients with IDA. Conclusion: In summary, the study shows that the pencil cells have a good correlation with TIBC and ROW. In addition, pencil cells and cigar cells can be used in ruling in a patient into iron deficiency anaemia
