Isi artikelBackground: Extubation as well as intubation often provokes cardiovascular changes with marked increased in arterial blood pressure and heart rate. In such patients this cardiovascular changes may be tolerable, but certain patients with coexisting disease may be unable to tolerate this response and experience undesirable squeal. Lidocaine with its effect on Na-channel and Ca-¬channels can attenuate this response. Objective: To compare the effectiveness of intratracheal lidoacaine and intravenous lidocaine for suppressing homodynamic response to tracheal extubation Method: Forty-two ASA I patients, underwent elective or emergency orthopedic, gynecologic, lower abdominal and superficial surgery, in randomized divided into 2 groups. Group A receive 1 mg/kg intratracheal lidocaine 20 minutes before extubation and group B receive 1 mg/kg intravenous lidocaine 1-2 minutes before extubation. Systolic blood pressure, mean arterial pressure, diastolic pressure and pulse rate were measured at preoperational visit, before extubation, during extubation, 1 minute and 5 minutes after extubation. Result: There were no significant differences in homodynamic response or extubation scores between groupA and group B Conclusion: Intratracheal lidocaine 1 mg/kg administered 20 minutes before extubation in suppressing homodynamic response to tracheal extubation
