Bakcground: Optimal passive dilatation of gallbladder is required to obtain good visualization of gallbladder with a fasting period before ultrasound examination. Long fasting maybe inconvenient for some patients. Objectives: The objective of the study is to compare the ultrasonic appearance of gallbladder with 3 hours fasting and a minimum 6 hours prior to ultrasound examination. Optimal passive dilatation of gallbladder is required to obtain good visualization of gallbladder with a fasting period before ultrasound examination. Methods: This is an observational analytic study with cross sectional design. There are 160 patients consist of 80 patients in each study group. The group on a fasting period of 3 hours is test group whereas the group with minimum 6 hours fasting period as control group. Chi¬-square test is used for analysis. Result and Conclusion: Visualizations of gallbladder are significantly different between the group with a minimum 6 hours fasting period compare to the group with 3 hours fasting period (p = 0.001). Better visualizations of gallbladder are found in a greater rate in the group with a minimum 6 hours fasting period compare to the group with 3 hours fasting period [42(52.5%) vs 18(22.5%)]. Inadequate visualizations of gallbladder are shown in a higher rate in the group with 3 hour fasting period compare to minimum 6 hours fasting period [62(77.5%) vs 38(47.5)]. Preparation of a minimum 6 hours fasting prior to ultrasound examination provides better visualization of gallbladder than 3 hours fasting
