Background: In 2000 the IOM published a report entitled To err is human, building a safer health. The publication arouses awareness about the importance of patient safety problems in health services. Limitations in the field of health services in developing countries make the population at risk of facing the threat of safety greater than the population in developed countries. The challenges faced thus is how patient safety issues can be pursued effectively and efficiently together with resolve service issues that still have not been resolved. Objective: This paper describes the essential notions in the context of patient safety. Proposed conceptual framework can be used as the basis for implementation of patient safety in health care. Aspects that can be done to achieve patient safety goals are also discussed. Discussion: Patient safety in health services should become the responsibility of all stakeholders ranging from patient, care team, facility management services, to the environmental context. There are five areas that need to be focused, namely: governance, leadership, and culture; involving patients, families, and communities; competence and education of human resource; management information and reporting; and work environment. The stakeholders need to implement appropriate processes to enhance the quality dimensions that include safety, effectiveness, patient focus, timeliness, efficiency, and fairness. Conclusion: Implementation of the concept of patient safety in health services is an ongoing process that is comprehensive, involving all stakeholders and covers all aspects of service.
