Internal fixation is one of the treatment modalities in fracture management. Recently, the early and de¬layed internal fixations still become debatable topics. Internal fixation will trigger macrophage to express inflammatory mediator from activated macrophage. The aim of this study was to know the effect of internal fixation on systemic inflammatory response (IL-6). The design of the study was cross sectional. All study activities were carried out at Central Hospital Denpasar. Samples of this study were 40 cases of closed femoral fractures with internal fixation treat¬ment. Materials that were examined include patient serum taken before and after internal fixation for examining IL-6 by ELISA method. The result showed that internal fixation caused a significant increase of IL-6 (p<0.01).
