Esophageal atresia (EA) is the most frequent congenital anomaly of the esophagus. EA with tracheoesophageal fistula (TEF) or "type C" TEF is the most common form of EA. This type occurs in approximately 1 in 4000 live births and is associated with other congenital anomalies in 30-50% of the cases. It is classically suspected in cases of maternal polyhydramnios, when the infant has excessive oral secretions or develops choking, cyanosis, or coughing during feeding. The condition is confirmed by inability to pass a feeding tube in the stomach. Pulmonary aspiration of pharyngeal secretions with EA and aspiration of gastric contents via a distal fistula cause chemical pneumonitis. Early diagnosis is essential if aspiration pneumonitis is to be avoided and corrective surgery performed before pulmonary complication occurs. We report a case of EA with TEF and palatoschizis in a two day old infant. This case presented with an excessive vomiting which contain of foaming oral secretions from the first hour of live. This condition was worsening by difficult to breath in the second day. After failure to pass a feeding tube in the stomach, this condition was confirmed by the chest x-ray, plain abdominal x-ray and also by Barium intake. The x-ray finding showed that there was aspiration pneumonia and EA with TEF. Ligation of the TEF and end-¬to-end anastomosis of the esophagus was done. Sadly, although we have given the antibiotic and did the fluid, electrolyte and nutrition correction, the patient was going worst everyday and finally died after six days in neonatal care unit because of sepsis.
