Background: Noise Induced Hearing Loss (NIHL) is a hearing loss due to long-term exposure to noise. In industriol environment, NIHL is in first place compare to another occupational disease. However studies on prevelance of Noise Induced Hearing Loss (NIHL) ocaused by locomotive machine are in fact very rare. This study was conducted to determine the prevalence of NIHL for machinist and to find out the correlation between NIHL and working perod and machinist age. Design and Method: This study was analytic descriptive with Cross Sectional Design. Machinists of P. T Kereta Api Daop IV Semarang who fulfilled the inclusion and exclusion criteria were tested the audiometric after 12 hours free of noise. Result: The prevalence of NIHL among machinists at P. T. Kereta Api was 20.4%. Prevalence was found to be higher (18.9% or 26 workers) among the engineers serving for more than or equal to 10 years as compared to those who served less than 10 years (1.4% or 2 workers). The prevalence for the age group of less than 40 year, 40-50 year, and above 50 year were 4 workers (2.9%), 10 workers (7.3%), 14 workers (10.2%, respectively. Relative risk of NIHL prevalence on machinists after serving for 10 years increased 4 times. Conclusion: There was significant correlation between prevalence of NIHL and working period and tendency of the increase in NIHL with age
