Epistaxis or nosebleed which has been a challenging problem for our ENT specialty, specially intractable posterior epistaxis despite conservative measures. As trans nasal endoscopic sinonasal surgery has been a widely accepted technique nowadays. Conventional management options in the form of anterior and posterior packing, arterial ligation of the internal maxillary or the external carotid artery and embolization, are not entirely satisfactory because of morbidity, high failure rates and Significant complication. Our experience of endoscopic cauterization of sphenopalatine artery for two patients with per¬sistent posterior epistaxis is described, which was rapidly controlled following the procedure. This technique is simple and effective for persistent posterior epistaxis with low morbidity and complication. and should be one of the treatment options to be considered relatively early in the management of epistaxis refractory to anterior and posterior nasal packing.
