A Study on the Efficacy and Safety of Intravenous Ketorolac for Postoperative Pain Relief after General Anaesthesia for Open Reduction and Internal Fixation of Fracture Humerus

This study compared the analgesic efficacy and safety of I.V ketorolac 30 mg 6 hourly With that of PR diclofenac 50 mg 8 hourly for postoperative pain relief within 24 hr period, in patients undergoing ORIF of fracture humerus under general anesthesia. Sixty patients (thirty in each group) with ASA physical status I or II. age ranging from 18-60 yrs were studied. Supplementary analgesia (i.v tramadol 50 mg) was administered when VAS was = 6. VA (visual analogue scale. 10 cm linear scale). VRS (verbal rating scale), haemodynamic parameters and side effects of the patients were assessed 4 hourly during 24 hr post operative period starting at time of wound closure, and compared statistically between groups. Mean VAS scores in ketorolac group were lower than those of diclofenac group except at 24 hr assessment. The differences were statistically significant at 4,8 and 12 hr assessments. (P<0.05). Number of patients requiring rescue analgesia diclofenac group were greater than in ketorolac group, but the differences were not statistically significant (P=0,190). Rescue analgesic consumption of each patient in both groups was equal. Mean duration from the time of wound closure to the time of first rescue analgesic in ketorolac group was longer than that of diclofenac group and the difference was statistically significant, (P<0.001 ). In both groups, cardiopulmonary status was stable during 24 hr postoperative period and the risk of serious adverse effects with ketorolac was not different to that from diclofenac. Therefore, i.v ketorolac is an alternative analgesic agent to PR diclofenac in the treatment of moderate to severe postoperative pain.
