Simulation is widely used in medical education. One of these is case-based simulation. FM-GMU is one of medical institution that employs this technique. This technique is believed meet the requirement of Problem Based Learning method. PBL main characteristic is using health care problem analysis as the major technique to obtain and relate basic and clinical knowledge. The paper is aimed to explore the use of case-based simulation in Problem Based Learning context especially in FM-GMU. This exploration is based on my experience as one of Faculty member supported by information obtains from colleague and students' evaluation, I develop this narrative paper. The advantage of case-based simulation is the scenario can be developed according to the students' learning need. Further it is proof by previous research that case-based simulation is a good way for clinical decision learning. In the context of Problem Based Learning, case-based simulation is suitable in the form of scenario for tutorial discussion. It can trigger students learning and reflect their own knowledge and experience. To maintain quality of the case-based scenario, the case~based simulation should fulfil the content and construct validity. Innovation in doing scenario based on the case is widely open. Continuous evaluation is a must in maintaining the quality and the continuity of the case-based simulation.
