The Effect of Pancreatic Enzyme Inhibitor to Prevent
Multiple Organ Failure Caused by Shock

Shock and multiple organ failure are connected through a mechanism called autodigestion. Pancreatic digestive enzymes carried in the small intestine are a major cause of multiple organ failure in this case. Under normal circumstances, the digestive enzymes are compartmentalized inside the intestinal lumen, but under ischemic condition, the mucosal barrier of the wall breakdown. Following the breakdown, those enzymes leak into the wall of the intestine and start an autodigestion process that includes destruction of the villi in the intestine. Pancreatic digestive enzyme also generate cytotoxic mediators, which will be transported into the systemic circulation via vena porta system and lympahtics system. Those cytotoxic mediators will generate a systemic inflammation and cause various degrees of cell and organ dysfunction that can reach to the point of complete organ dysfunction.
Based on the autodigestion mechanism, several clinical experiments were developed to inhibit the works of the digestive enzymes. This literature will discuss about the effect of giving the inhibitor of pancreatic enzymes to induced shock models to prevent the autodigestion mechanism, and therefore preventing multiple organ failure. Several clinical studies and experiment show a very positive result based on the mice models whereas the mortality rate reached 0%. Hopefully this positive result will be taken to the next level which is experiment on human models thus improving the survival rate of the shock patient.
