Isi artikelA 37 y old woman was admitted to the Hospital in 1996 with a fever of 39°C, widespread erythema of both upper eye-lids, the neck and periumbilical area, red erythematosus plaques on the distal lower extremities, deep ulcerations of the elbows and soles, and conjunctival injection without discharge. She had four febrile episodes in the last ten years and she carried a diagnosis of Weber-Christian disease. Ten years ago, the first febrile episode was characterised by bilateral cervical and inguinal lymphadenopathy, non-tender erythematous subcutaneous nodules in the inner surface of the distal third of the lower extremities and a murmur over the precordium. The diagnosis of rheumatic fever was made, and intramuscular penicillin was given monthly for two years. The patient was relatively well until five years before admission, when she experienced a second episode of fever and erythematous subcutaneous nodules in the distal portions of the lower extremities. Broad spectrum antibiotics were administered for one month with no improvement.
