I have been asked to address the treatment of patients with cutaneous lupus erythematosus who fail to respond to antimalarial therapy. In order to clearly address this issue, I feel that I must first define what antimalarial-refractory cutaneous lupus erythematosus is. Patients who have been treated with ’standard’ therapy including the regular use of an appropriate sunscreen, avoidance of sun or ultraviolet exposure, application of an appropriate topical corticosteroid agent, and at least eight weeks of antimalarial therapy who continue to develop new lesions, or whose lesions remain active can be considered to be antimalarial-refractory.
