Isi artikelWith all respect to recent arguments to the contraryl°2, the simple truth is that cyclophosphamide has become an important drug in the treatment of severe lupus. By every indication, the drug is widely used and most would regard it as the chemotherapy drug of first choice in the management of seriously ill patients with lupus. In lupus nephritis, longterm randomized trials have clearly demonstrated that intermittent, bolus therapy prevents the loss of renal function and reduces the risk of end-stage renal failure 3,4 The economic consequences of the latter are far from trivial with projections that millions of dollars are saved annually through the delay or prevention of hemodialysis or renal transplantations. The effects of bolus cyclophosphamide on other serious manifestations of lupus have been less well studied, although there is now a rather substantial clinical literature describing benefits in hematologic, cardiopulmonary and neurologic disease featureS6.
