This article discusses the methodologies available through hospital laboratories or commercial clinical laboratories for drug abuse screening. The relative sensitivity, specifically of each methodology, causes of false-positive and false-negative testing, and the duration of expected positivity of a test after exposure to a drug are reviewed. A discussion of testing adolescents for anabolic steroid use is also included. Ethical, legal, and medical considerations to be weighed by the pediatrician who is considering ordering a drug abuse screen are presented. Pediatricians should be familiar with the capabilities and limitations of laboratory testing so as to enhance their expertise in addressing substance abuse concerns of children, adolescents, and their families.
