In economic crisis situation the country budget for malaria control program is very limited, therefore case finding and treatment activities are the most reeeasonable choise for this program to reduce transmission of the disease. Community empowerment is one option to intensify these activities, for increasing case finding and treatment coverage which prevents malaria transmission. This is an intervention study which include health, education, training, and volunteer's recruitment for case finding and treatment. The evaluation were done on 6, 12 and 18 months after intervention study in Way Muli village and Rajabasa village as a control area. The slide positive rate (SPR) in the community and health center, monthly malaria incidence (MOMI), the coverage of case finding and treatment by cadre, and outbreak occurrence were the parameters in this study. Even thoughh there were obstacles faced by cadre in accomplishing of their work, with in 18 months the SPR significantly decreased from 18,9% (66/348) to 1,9% (8/425) (p<0,05) in the intervention area. Meanwhile the SPR in control area relatively still high 14,4% (29/201) to 13,7% (30/219) (p> 0,005). Monthly malaria incidence in intervention area was also lower than in control area 5,4% versus 24% in November 2004, furthermore outbreak was not occurred during evaluation periods. It was concluded that participation and awareness of the community not only the cadres, could reduce malaria incidence significantly. Even though number of cases was increased in certain period, there was no outbreak reported.
