Abstract
Background, HIV-positive women suffer from psychosocial burden in dealing with HIV/AIDS infection and its treatment. The way of thinking and irrational beliefs of HIV-positive women about infections, psychosocial impact of HIV/AIDS and antiretroviral treatment can lead to emotional and mental health disorder and affect ART adherence. Rational Emotive Behavior Therapy (REBT), which modifies irrational beliefs into rational belief, helps people infected with HIV by forming optimal self-realization and establishing rational behavior and emotion, together with ART adherence. This study aims to prove that REBT can raise ART adherence with the improvement of irrational beliefs (KI) and mental health (SRQ-20) in HIV-positive women.

Methods: This experimental research tool place in ART service in Cipto Mangunkusumo Hospital and Dharma Hospital from October 2011 until March 2012. The sampling method used was consecutive sampling. A total of 160 women infected with HIV through sexual intercourse with their husbands'male partners participated in this experiment. These participants were randomized into REBT intervention group (n=80) and control (n=80). ART adherence, SRRQ-20, KI, and the risk factors, which are assumed able to modify the effect of REBT on ART adherence, were analyzed using the Generalized Linear Model (GLM) and the Generalized Estimating Equation (GEE). Validation test on KKI and Diagnostic test on KPKA as measuring instruments wene done.

Results. Validation test has proved that KKI is a reliable measuring instrument with Cronbach's alpha coefficient of 0.85. KPKA diagnostic test has shown sensitivity of 94 % and a specificity of 89 %. GLM analysis showed a decrease in irrational beliefs mean scores, SRQ- 20 mean score and an increase in the adherence mean scores on the REBT group is more significant (p <= 0.00) than the control group. There were not any risk factors that significantly affect REBT on ART adherence. GEE analysis showed the decrease in the irrational beliefs (KI) mean score that affects the decrease in SRQ-20 mean score of REBT group is more significant (p < 0.05) than the control group and the decrease in the SRQ-20 mean scores that affects the increase in the average ART adherence scores in the REBT group is more significant (p <= 0.00) than the control group.

Conclusion. REBT reduces the irrational beliefs score, followed with a decrease in SRQ-20 scores and increase in ART adherence scores of HIV-positive women. KKI and KPKA in Indonesian language are valid and can be widely used.
